Who are we?
Vermont Comforts of Home program is jointly marketed by two Vermont organizations: Lincoln Street Inc. and Upper Valley Services, Inc. Both organizations are experienced in the provision of quality individualized home care. We each offer support to individuals who qualify for a nursing home level of care within our designated geographic areas. The goal of Vermont Comforts of Home is to provide a choice when, due to medical and personal care needs, a person can no longer live at home without support. Our collective experience has demonstrated most individuals can continue to live in a home environment with appropriate care.
Each of Lincoln Street Inc. and Upper Valley Services, Inc. have been leaders in the field, providing high quality community residential support to individuals with disabilities for the past 40 years. The two organizations have joined together to collectively market their respective expertise under the name Vermont Comforts of Home to give older Vermonters and others requiring nursing home level of care a choice in where they receive those services.


What are our values?
Each of our two organizations are committed to the belief that all people, regardless of their age or care needs, should have a voice in how they are supported and where they live. Working with you, one of our organizations will ensure that all services are individualized to meet the specifics needs of those we support.
What services do we provide?
- Care coordination: Working with you, the organization providing you with services develops an individualized plan of care that addresses specific care needs, medication requirements, support and oversight of residential services.
- Residential support: The organization providing you with services contracts with skilled providers in the community who share their home and provide individualized care.
- Safety: The safety of those supported by the program is of the utmost importance. A thorough background screening is completed on all providers as well as a home safety inspection.
The services offered under Vermont Comforts of Home operate in compliance with the regulations of the Adult Family Care program under Vermont’s Choices for Care initiative. Services are provided by either Lincoln Street, Inc. or Upper Valley Services, Inc. to individuals who qualify for Medicaid and those interested in a private pay option.
